ABSTRACT

ObjectiveTo summarize the characteristics of traditional Chinese medicine (TCM) syndrome in primary Sjögren's syndrome (pSS) patients with interstitial lung disease (ILD) and to explore associated factors. MethodA survey was conducted and pSS patients who were treated in TCM department of rheumatism at China-Japan Friendship Hospital from December 2018 to April 2022 were included. Tongue manifestations and syndromes of patients were recorded. pSS patients with ILD were classified into the pSS-ILD group and those without the ILD were included in the pSS-non-ILD group. The tongue manifestations, syndromes, and laboratory indexes were compared between the two groups, and logistic regression was used to explore the factors associated with pSS-ILD. ResultA total of 200 pSS patients were included, with 186 (93.0%) females, median age of 57 years, and median disease course of 60 months, of which 44 (22%) had pSS-ILD. In terms of tongue manifestations, pSS-ILD patients generally had dark/purple/stasis tongue, fissured tongue, and tongue with little fluid, thick coating, yellow coating, and greasy coating. The proportion patients with yellow coating was higher in pSS-ILD group than in the pSS-non-ILD group (χ2=4.799,P<0.05). In terms of syndrome, more than 40% of pSS-ILD patients had Qi deficiency, Yin deficiency, phlegm-dampness, Qi stagnation, and/or blood stasis syndrome. As for Yin deficiency, liver-kidney Yin deficiency syndrome ranked the first. For Qi deficiency, lung Qi deficiency syndrome was most commonly seen. The proportion of patients with lung Qi deficiency was higher in the pSS-ILD group than in the pSS-non-ILD group (χ2=18.667,P<0.01). As to laboratory indexes, compared with the pSS-non-ILD group, pSS-ILD group had high proportion of anti-SSA-positive patients (P<0.05) and high levels of C-reactive protein (CRP) (P<0.01), complement C3 (χ2=4.332,P<0.05), and complement C4 (P<0.05). Logistic regression analysis showed that pSS with ILD was positively associated with lung Qi deficiency [odds ratio (OR)=6.079, 95% confidence interval (CI) 2.585-14.298, P<0.01)] and yellow coating (OR=5.260, 95% CI 1.337-20.692, P<0.05) and negatively associated with low C4 (OR=0.199, 95% CI 0.070-0.564, P<0.01). ConclusionAbout 22% of pSS patients had ILD, and patients with pSS-ILD generally have Qi deficiency, Yin deficiency, phlegm-dampness, Qi stagnation, and/or blood stasis syndrome. Yellow coating, lung Qi deficiency and C4 level are factors associated with pSS combined with ILD.

ABSTRACT

ObjectiveTo observe the efficacy and safety of Huashi Runzao prescription for patients with primary Sjögren's syndrome (pSS) of combined dryness and dampness pattern. MethodA total of 105 eligible patients were randomized into the experimental group (65 cases) and control group (40 cases), and they were respectively treated with Huashi Runzao prescription and hydroxychloroquine for 12 weeks. Visual Analogue Scale (VAS) was employed to assess the symptoms. The symptoms of dryness, fatigue, and pain, European League Against Rheumatism (EULAR) Sjögren's Syndrome Patient Reported Index (ESSPRI), EULAR Sjögren's syndrome disease activity index (ESSDAI), and immune inflammatory indicators before and after treatment were compared between the two groups, and adverse reactions were observed. ResultAfter treatment, the ESSPRI score was lower than that before treatment in the experimental groups (P<0.01) and was lower in the experimental group than in the control group (P<0.05). The VAS scores of dry mouth, dry eyes, overall dryness, fatigue, and pain in the experimental group decreased compared with those before treatment (P<0.01), and the experimental group had lower VAS scores of dry mouth and overall dryness than the control group (P<0.01). After treatment, the ESSDAI score of both groups decreased compared with that before treatment (P<0.05, P<0.01), but there was no significant difference between the groups. After treatment, the level of immunoglobulin M (IgM) decreased (P<0.01) and the level of complement C3 increased (P<0.01) in the experimental group, while the level of complement C3 decreased in the control group (P<0.05). There was no significant difference in the laboratory indexes between groups. During the treatment, stomachache occurred to one case in the experimental group, which was alleviated after the treatment, and no adverse reaction was observed in the control group. According to the chi-square test, the occurrence of adverse reactions was insignificantly different between the two groups. ConclusionHuashi Runzao prescription can alleviate the symptoms of dryness, fatigue, and pain, and reduce disease activity without associated side effects in pSS patients with combined dampness and dryness pattern.

ABSTRACT

ObjectiveTo investigate the effects of Huashi Runzao prescription (HRP) on the histopathological injury and function of submandibular gland in naive non-obese diabetic (NOD/Ltj) mouse model of Sjögren's syndrome (SS) and its regulatory effect on aquaporin 5 (AQP5) expression in submandibular gland cells. MethodThe SS model was induced in NOD/Ltj mice. The NOD/Ltj female mice aged nine weeks were selected and randomly assigned into model group,HRP group (7.15 g·kg-1·d-1),and hydroxychloroquine (HCQ) group (1.30 g·kg-1·d-1), and female BALB/c mice in the same age were selected and assigned into the normal group, with six mice in each group. Drug intervention lasted eight weeks. The water consumption and salivary flow rate (SFR) of each group were recorded. The pathological staining results of the submandibular gland of mice in each group were observed and scored. AQP5 expression was determined by immunohistochemistry (IHC) and Western blot. ResultCompared with the normal group, the model group showed increased water consumption (P<0.05) and reduced SFR (P<0.05). Compared with the model group, the HRP group showed decreased water consumption (P<0.05) and increased SFR (P<0.05), and the HCQ group showed increased SFR (P<0.05). In terms of histopathological results of the submandibular gland,compared with the normal group,the model group showed increased pathological score, number of lymphocyte infiltration foci,and percentage of lymphatic infiltration area (P<0.05). Compared with the model group, the HRP group showed reduced pathological scores and number of lymphocyte infiltration foci (P<0.05), and the HRP group and the HCQ group showed reduced percentage of lymphatic infiltration area(P<0.05). The results of IHC and Western blot showed that compared with the normal group,the model group showed down-regulated expression level of AQP5 protein (P<0.05), and compared with the model group and the HCQ group,the HRP group showed up-regulated expression level of AQP5 protein (P<0.05). ConclusionHRP can improve the secretion function of submandibular gland acinous cells and glandular structure injury in SS model mice, and its mechanism may be related to the up-regulation of AQP5 protein expression level in submandibular gland cells.

ABSTRACT

Five new flavonoid derivatives, cajavolubones A-E (1-5), along with six known analogues (6-11) were isolated from Cajanus volubilis, and their structures were elucidated by spectroscopic analysis and quantum chemical calculations. Cajavolubones A and B (1 and 2) were identified as two geranylated chalcones. Cajavolubone C (3) was a prenylated flavone, while cajavolubones D and E (4 and 5) were two prenylated isoflavanones. Compounds 3, 8, 9 and 11 displayed cytotoxicity against HCT-116 cancer cell line.

ABSTRACT

Objective:To correlate climate change with epistaxis in Yueqing of Zhejiang province and to provide evidence for the diagnosis and treatment of epistaxis.Methods:A total of 1 800 patients with epistaxis, who received treatment in Yueqing People's Hospital between October 2018 and October 2019, were included in this study. The data of these patients were input into the Microsoft Excel software. They were summarized and sorted as per admission time. Climate change was correlated with epistaxis.Results:Daily average temperatures were negatively correlated with epistaxis ( r = -0.65, P = 0.003). A non-linear, inverted-U-shaped relationship was observed between daily relative humidity and epistaxis. When the daily relative humidity was < 65%, daily relative humidity was positively correlated with epistaxis ( r = -0.54, P = 0.007).When the daily relative humidity was > 65%, daily relative humidity was negatively correlated with epistaxis ( r = -0.68, P = 0.002). There was a nearly linear positive correlation between diurnal temperature range and epistaxis ( r = 0.52, P = 0.009). There was a linear and positive correlation between daily average atmospheric pressure and epistaxis ( r = 0.60, P = 0.004). The risk of epistaxis increased by 1.48% (95% CI: -2.15 to -0.81) when the temperature decreased by 1 ℃. When daily relative humidity was < 65%, the effects of humidity change on the risk of epistaxis were not statistically significant ( P > 0.05). When the relative humidity of the day was > 65%, the risk of epistaxis decreased by 1.82% (95% CI: -2.71 to -0.93) for every unit of humidity rise. The risk of epistaxis increased by 2.86% (95% CI: 0.54 - 5.18) for every 1 ℃ increase in temperature. The risk of epistaxis increased by 1.18% (95% CI: 0.50 - 1.87) for every 1 Pa increase in air pressure. Conclusion:Temperature change is negatively correlated with epistaxis. Atmospheric pressure and diurnal temperature range are positively correlated with epistaxis. Temperature change, atmospheric pressure, and diurnal temperature have temporary effects on epistaxis. High humidity has an obvious long-term protective effect against epistaxis.

ABSTRACT

Fourteen new geranyl phenyl ethers (1-14) along with three known compounds (15-17) were isolated from Illicium micranthum, and their structures were elucidated by comprehensive spectroscopic methods. Illimicranins A-H (1-8) were characterized as geranyl vanillin ethers, while 9 and 10 were dimethyl acetal derivatives. Illimicranins I and J (11 and 12) were rare geranyl isoeugenol ethers. Illimicranins K and L (13 and 14) represented the first example of geranyl guaiacylacetone ether and geranyl zingerone ether, respectively. Compounds 1, 2 and 15 exhibited anti-HBV (hepatitis B virus) activity against HBsAg (hepatitis B surface antigen) and HBeAg (hepatitis B e antigen) secretion, and HBV DNA replication.

ABSTRACT

OBJECTIVES@#Genetic mutation is one of the important causes for tumor genesis and development, but genetic mutation in nasopharyngeal carcinoma (NPC) has rarely been reported. This study explored the role of phosphatidylinositol 3 kinase-protein kinase B (PI3K-Akt), mammalian target of rapamycin (mTOR), and adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK) signaling pathway in the efficacy and prognosis in patients with NPC.@*METHODS@#A total of 31 patients with advanced NPC, who came from the Affiliated Cancer Hospital of Xiangya School of Medicine of Central South University/Hunan Provincial Cancer Hospital, were enrolled. All of the exons of 288 genes, introns of 38 genes and promoters or fusion breakpoint regions from the nasopharyngeal biopsy tissues before treatment were detected by the gene sequencing platform Illumina NextSeq CN500. The coding regions of 728 genes were carried out a high-depth sequencing of target region capture, and the 4 variant types of tumor genes (including point mutations, insertion deletions of small fragments, copy number variations, and currently known fusion genes) were detected. All of 31 patients received platinum-based induction chemotherapy combined with concurrent chemoradiotherapy and were followed up for a long time.@*RESULTS@#The 3-year regional failure-free survival (RFFS) and disease-free survival (DFS) in patients with PI3K-Akt pathway mutation were significantly lower than those in unmutated patients (χ2=6.647, P<0.05). The 3-year RFFS and DFS in patients with mTOR pathway mutations were significantly lower than those in unmutated patients, and there was significant difference (χ2=5.570, P<0.05). The rate of complete response (CR) in patients with unmutated AMPK pathway was significantly higher than that in patients with mutation at 3 months after treatment (P<0.05), and the 3-year RFFS and DFS in patients with AMPK pathway mutation were significantly lower than those in unmutated patients (χ2=4.553, P<0.05). PI3K-Akt/mTOR/AMPK signaling pathway mutations and pre-treatment EB virus DNA copy numbers were independent prognostic factors for 3-year RFFS and DFS in patients with NPC (both P<0.05).@*CONCLUSIONS@#The NPC patients with PI3K-Akt/mTOR/AMPK signaling pathway mutation have poor prognosis, and the detection of PI3K-Akt, mTOR, AMPK driver genes and signaling pathways by next-generation sequencing is expected to provide new idea for basic research and targeted therapy of NPC.

ABSTRACT

Objective:Chemiluminescence immunoassay was used to detect the levels of anticardiolipin antibody (aCL) -IgA/IgG/IgM and anti-β2-glycoprotein Ⅰ antibody (aβ2GPⅠ) -IgA/IgG/IgM in healthy non-pregnant and pregnant women to explore the changes of antiphospholipid antibody in different pregnancy periods.Methods:This prospective study was conducted in Shandong Provincial Maternal and Child Health Care Hospital Affiliated to Qingdao University, involving normal pregnant women who underwent prenatal examination and healthy non-pregnant women with no history of adverse pregnancy who underwent progestational eugenic health examination from April 2020 to August 2021. The levels of aCL-IgA/IgG/IgM and aβ2GPⅠ-IgA/IgG/IgM were detected using BIO-FLASH chemiluminescence immunoassay analyzer and P95 as well as P99 were calculated, respectively. The difference in the six data between non-pregnant and pregnant women was compared using Mann-Whitney U test. Kruskal-Wallis H test was used to compare the change of each antibody in different pregnancy periods and Spearman correlation was used to analyze the correlation between different trimester and the levels of aCL-IgA/IgG/IgM and aβ2GPⅠ-IgA/IgG/IgM. Results:A total of 454 cases met the inclusion criteria, and 435 cases were included in the analysis after excluding 19 cases, among them 110 were non-pregnant women and 325 were pregnant women, including 110 cases in the first trimester (≤13 +6 weeks), 110 cases in the second trimester(14 +0-27 +6 weeks), and 105 cases in the third trimester (≥28 weeks). P99 value of aCL-IgA/IgG/IgM and aβ2GPⅠ-IgA/IgG/IgM in the non-pregnant women were 7.31, 14.70, 7.92, 3.58, 13.60, and 4.95 CU, which in the pregnant women were 5.90, 12.78, 5.70, 1.60, 10.65, and 3.90 CU, and were all lower than the cut-off value of 20 CU that given by the analyzer manufacturer. The levels of aCL-IgA/IgG/IgM, and aβ2GPⅠ-IgG/IgM in the pregnant women were significantly decreased comparing with the non-pregnant women [aCL-IgA: 1.90 CU (1.40-2.70 CU) vs 2.90 CU (2.20-3.83 CU), Z=-7.14; aCL-IgG: 3.00 CU (2.20-4.50 CU) vs 6.10 CU (4.20-7.83 CU), Z=-10.26; aCL-IgM: 1.40 CU (1.10-2.30 CU) vs 2.65 CU (2.08-3.73 CU), Z=-8.87; aβ2GPⅠ-IgG: 3.50 CU (2.60-4.90 CU) vs 4.75 CU (3.60-5.93 CU), Z=-5.45; aβ2GPⅠ-IgM: 0.70 CU (0.50-1.20 CU) vs 1.00 CU (0.60-1.53 CU) , Z=-3.73; all P<0.001]. The aCL-IgA level in the third trimester was higher than those in the first and second trimester (both P<0.05). The levels of aCL-IgG/IgM in the second trimester and aβ2GPⅠ- IgG in the second and third trimesters were significantly decreased than those in the first trimester (all P<0.05). Spearman analysis showed that aCL-IgG/IgM, aβ2GPⅠ-IgA/IgM had no significant correlation with the pregnancy period (the first, second and the third trimester) (all P>0.05). However, a weak correlation between the aCL-IgA, aβ2GPⅠ- IgG and the pregnancy period was observed ( r=0.28 and-0.49, both P<0.001) Conclusions:P99 value of aCL-IgA/IgG/IgM and aβ2GPⅠ-IgA/IgG/IgM levels in normal pregnant women and non-pregnant women are lower than the cut-off value of 20 CU given by the analyzer manufacturer. The levels of aCL-IgA/IgG/IgM and aβ2GPⅠ-IgG/IgM during pregnancy are lower than those before pregnancy and fluctuate with the pregnancy period, but have no significant correlation with the pregnancy period. The clinical diagnosis of antiphospholipid syndrome should be made according to the cut-off values of aCL-IgA/IgG/IgM and aβ2GPⅠ-IgA/IgG/IgM determined by each laboratory.

ABSTRACT

Objective:To analyze the application of video nystagmography in the diagnosis and treatment of benign paroxysmal positional vertigo (BPPV) so as to provide evidence for clinical diagnosis.Methods:A total of 120 patients with suspected BPPV who received treatment in Yueqing People's Hospital from January to July 2020 were included in this study. There were anterior ( n = 24), posterior ( n = 80) and horizontal semicircular canal ( n = 16) BPPV according to the disease type. The detection rates of anterior, posterior and horizontal semicircular canal BPPV by video nystagmography and naked eyes were determined. Therapeutic effects of video nystagmography on three types of BPPV were compared. Results:The diagnostic rates of anterior, posterior and horizontal semicircular canal BPPV were 91.68%, 92.50% and 93.75%, respectively, and the difference was not statistically significant ( χ2 = 0.06, P > 0.05). The detection rates of anterior, posterior and horizontal semicircular canal BPPV by video nystagmography were 91.68%, 92.50% and 93.75% respectively, which were significantly higher than those by naked eyes (58.34%, 78.75%, 56.25%, χ2 = 7.11, 6.14, 3.86, all P < 0.05). After 1 week of treatment, total response rates of anterior, posterior and horizontal semicircular canal BPPV were 62.50%, 66.25%, 68.75%, respectively. After 3 months of treatment, total response rates of anterior, posterior and horizontal semicircular canal BPPV were 95.83%, 96.25% and 100.00%, respectively. There were no significant differences between anterior, posterior and horizontal semicircular canal BPPV after 1 week and 3 months of treatment ( χ2 = 0.18, 0.64, P > 0.05). At 3 months after surgery, the recurrence rates of anterior, posterior and horizontal semicircular canal BPPV were 4.16%, 5.00% and 6.25%, respectively, and the difference was not statistically significant ( χ2 = 0.08, P > 0.05). Conclusion:Video nystagmography is highly effective in the diagnosis and treatment of BPPV. It can accurately judge the occurrence of nystagmus in BPPV and increase the detection rate, which is worthy of clinical application.

Based on the theory of traditional Chinese medicine and combined with modern pharmacological research ,this paper reviews single ingredient and active components of TWBXD ,calming and tranquilizing role of related couplet medicinals . The results showed that the single ingredients such as Rehmannia glutinosa ,the seeds of Zizyphus jujube ,the seeds of Platycladus orientalis ,Angelica sinensis,Schisandra chinensis ,Poria cocos and their effective components as well as related couplet medicinals of Z. jujube could improve the insomnia symptoms in different degrees . Except that the mechanism of the seeds of P. orientalis has not been studied , other ingredients mainly act on nervous -endocrine-immune system . They can play the sedative and hypnotic effects by regulating neurotransmitters,inflammatory factors and oxidation .

ABSTRACT

Objective To investigate the current status and its influencing factors of ART coverage and VL inhibition rate of HIV/AIDS in Yining City, Xinjiang, and to provide reference for AIDS prevention and treatment. Methods The ART data for 2017-2019 years in Yining, Xinjiang was retrieved from the National AIDS Antiretroviral Treatment Information System, to analyze the changing trend of ART coverage rate and VL inhibition rate of HIV/AIDS. Logistics regression model was used to analyze the influencing factors of untreated HIV/AIDS and uninhibited VL. Results The coverage rate of antiviral treatment in Yining city from 2017 to 2019 was 73.39%, 78.06% and 87.03%, respectively. The inhibition rates of VL were 83.91%, 85.05% and 86.09%, respectively, showing an increasing trend year by year. Female, other transmission routes, positive sexual partner detection, and testing and special investigation reduced the risks of untreated HIV/AIDS, while other domicile locations, non-marital and non-commercial heterosexual contact, and unawareness of their own VL increased the risks of untreated HIV/AIDS. Female, 26-35 years old, 36-45 years old, 46~55 years old, >55 years old, primary school, junior high school, high school or technical secondary school, junior college or above, and male-male sex behavior reduced the risks of unsuppressed VL of HIV/AIDS, while other domicile locations, non-marital and non-commercial heterosexual contact, and unawareness of their own VL increased the risks of unsuppressed VL in HIV/AIDS. Conclusion The ART coverage rate and VL inhibition rate in Yining, Xinjiang rise year by year, approaching the expected target. Targeted education and supervision should be conducted to promote the realization of “90-90-90” in 2020 and “95-95-95” target in 2030.

ABSTRACT

Objective:To detect the factors related to hemocytopenia and interstitial lung disease (ILD) secondary to primary Sj?gren's syndrome (pSS) in in-patients.Methods:Clinical characteristics of patients with pSS hospitalized in China-Japan Friendship Hospital from March 2014 to October 2020 were retrospectively analyzed. Patients were divided into different groups according to hemocytopenia and ILD. Chi-square test and Mann-Whitney U test were used to compare data in subgroups, and logistic regression were used to detect factors related to hemocytopenia and ILD secondary to pSS. Results:Five hundred and seventy-one inpatients with pSS were included in this study and the female: male ratio was 8∶1. Two hundred and seventy five of included patients had hemocytopenia (48.2%) and 180 patients had ILD (31.5%). Compared with patients without hemocytopenia, patients with hemocytopenia had higher ratio in low C3 [ OR=2.326, 95% CI(1.483, 3.650), P<0.01] and elevation of erythrocyte sedimentation rate (ESR) [ OR=1.912, 95% CI(1.233, 2.964), PP<0.01) . Compared with patients without ILD, patients with ILD showed higher ratio in male[ OR=2.509, 95% CI(1.269, 4.959), P<0.01) and fatigue [ OR=5.190, 95% CI(5.190, 13.931), P<0.01) , lower positive rate of anti-SSA antibody [ OR=0.392, 95% CI(0.230, 0.668), P<0.01) and anti-CENPB [ OR=0.337, 95% CI(0.145, 0.782), P<0.01] antibodies, and lower ratio in low C3[ OR=0.332, 95% CI(0.189, 0.582), P<0.01]. Conclusion:Low C3 and high ESR may be risk factors for developing hemocytopenia in patients with pSS. Men with pSS may be susceptible to ILD, while pSS patients with ILD may have lower ratio in positive anti-SSA antibody, positive anti-CENPB antibody and low C3. All of these findings are yet to be validated by future prospective cohort studies.

Paeoniae Radix Rubra is a traditional Chinese medicine commonly used in clinical practice, it is mostly wild and widely distributed in different areas of China. In addition, the plant of Paeoniae Radix Rubra also has ornamental value. Modern phytochemical researches showed that the chemical constituents of Paeoniae Radix Rubra were complex. Up to now, more than 300 chemical constituents have been found, mainly including monoterpene glycosides, triterpenoids, flavonoids, tannins, phenolic acids, saccharides, steroids, volatile oils and so on. Among them, the content of monoterpene glycosides was the highest, and the types of volatile oil were the most. Paeoniae Radix Rubra has a wide range of pharmacological effects, exerting different curative effects in multiple systems such as blood, cardiovascular, nervous and digestive system. It can protect myocardial cells and nerve cells, stabilize microcirculation, anti-endotoxin, anti-atherosclerosis, reduce pulmonary hypertension, anti-depression, protect liver, anti-gastric ulcer, anti-tumor, slow down aging, treat Parkinson's syndrome and diabetes and its complications, anti-radiation, anti-inflammatory, anti-virus and so on. Through reviewing the literature on chemical constituents and pharmacological effects of Paeoniae Radix Rubra, it was found that total glycosides and monomers such as paeoniflorin, albiflorin, benzoylpaeoniflorin and gallic acid may be the main active components of Paeoniae Radix Rubra. At present, the research on Paeoniae Radix Rubra mainly focused on monoterpene glycosides, while the research on flavonoids and volatile oil in Paeoniae Radix Rubra was less. It is suggested that research on these two components should be strengthened in the future.

ABSTRACT

Objective:To compare the clinical characteristics of patients with primary Sj?gren's syndrome (pSS) by gender and age of disease onset.Methods:Clinical data of hospitalized patients with pSS in China-Japan Friendship Hospital from March 2014 to August 2020 were retrospectively analyzed. Patients were divided into different groups according to their genders and ages of disease onset (young grong <30 years, middle-age group 30-59 years, elderly group ≥60 years). Chi-square test and Mann-Whitney U test were used to compare data in subgroups, and logistic regression was performed to analyze data after control covariates. Results:Five hundred and forty one patients with pSS were included in this study and 481 of them were women (88.9%) Women had higher ratio than men in dry mouth [ OR (95% CI) =2.172(1.091, 4.323), P=0.027], dry eyes [ OR(95% CI)=2.179(1.062, 4.469), P=0.034], leukocytopenia [ OR(95% CI)=3.789(1.150, 12.482), P=0.029], ANA titer≥1∶160 [ OR(95% CI)=2.233(1.279, 3.898), P<0.01], positive anti-SSA [ OR(95% CI)=2.919(1.644, 5.183), P<0.01], positive anti-Ro52 [ OR(95% CI)=3.018(1.685, 5.405), P<0.01], while men had higher ratio than women in parotid enlargement [ OR(95% CI)=2.345(1.281, 4.325), P<0.01], and interstitial lung disease (ILD) [ OR(95% CI)=2.593(1.460, 4.606), P<0.01]. Compared with patients in young group, patients in middles age group had higher ratio in dental caries [ OR(95% CI)=5.940(2.230, 15.819), P<0.01], xero-phthalmia [ OR(95% CI)=2.904(1.313, 6.425), P<0.01], arthralgia [ OR(95% CI)=1.959(1.039, 3.694), P=0.038] and ILD [ OR(95% CI)=2.247(1.018, 4.959), P=0.045], but lower ratio in renal involvement [ OR(95% CI)=0.402(0.211, 0.766), P<0.01]; patients in elderly group had higher ratio in dental caries [ OR(95% CI)=7.437(2.441, 22.656), P<0.01], xerophthalmia [ OR(95% CI)=6.084(1.901, 19.468), P<0.01], and ILD [ OR(95% CI)=4.857(2.029,11.627), P<0.01]. Compared with patients in elderly group, patients in young group had higher positive rate in anti-SSA [ OR(95% CI)=2.836(1.245, 6.459), P=0.013], anti-SSB [ OR(95% CI)=3.075(1.413, 6.690), P<0.01], rheumatoid factors (RF) [ OR(95% CI)=3.323(1.620, 6.817), P<0.01] and elevated immunoglobulin G (IgG)[ OR(95% CI)=3.567(1.747, 7.284), P<0.01]; patients in middle age group had higher positive rate in anti-SSB[ OR(95% CI)=2.330(1.315, 4.130), P<0.01], RF [ OR(95% CI)=2.026(1.279, 3.208), P<0.01], and elevated IgG [ OR(95% CI)=2.077(1.297, 3.327), P<0.01]. Conclusion:Women with pSS present a higher ratio in dry mouth, dry eyes, leukocytopenia and positive autoantibodies than men, while parotid enlargement and ILD are more common in men than women. Young patients are prone to renal involvement, middle-aged patients are prone to arthralgia, while elderly patients are prone to dental caries and xerophthalmia. With the growth of age, the positive rates of autoantibodies and elevated IgG are decreased gradually in patients with pSS, but the ratios of dental caries, xerophthalmia and ILD is increased gradually.

ABSTRACT

Objective:To probe the influence of obstruction sleep apnea syndrome (OSAS) on cardiac function and serum cystatin-C (CC) levels in patients with chronic heart failure (CHF), and to discuss its clinical significance.Methods:129 CHF patients suspected of OSAS were selected and divided into observation group (with apnea) and control group (without apnea) according to sleep apnea hypopnea index (AHI). The observation group included mild, moderate and severe groups. After admission, the levels of left ventricular ejection fraction (LVEF), N-terminal forebrain natriuretic peptide (NT-proBNP) and CC were collected for further analysis.Results:The levels of NT-proBNP and CC in 99 patients with CHF complicated with OSAS were higher than those in the control group, while LVEF was lower than those in the control group ( P<0.05); The levels of NT-proBNP and CC in severe group were higher than those in mild and moderate group, while LVEF was significantly lower ( P<0.05); The level of CC in mild to moderate group was higher than that in control group, while LVEF was lower than that in control group ( P<0.05). Rank correlation analysis showed that CC was positively correlated with AHI ( r=0.726, P<0.01); However, there was no significant difference in NT proBNP between mild to moderate group and control group ( P>0.05). Conclusions:OSAS can lead to the deterioration of cardiac function. The CC of CHF patients with OSAS is higher and increases with the aggravation of obstruction. CC may participate in the progression of the disease. A high level of CC may aggravate the development of the disease, and the incidence and mortality of cardiovascular events may also increase.Early prevention and treatment should be given.

ABSTRACT

Colorectal cancer is a malignancy with high mortality. Huangqin Tea(HQT) can exert potential preventive and therapeutic effects on colorectal cancer. Flavonoids are the main compounds in HQT, but the pharmacodynamic material basis and mechanism are unclear. Network pharmacology and molecular docking were used to predict and analyze the targets and signaling pathways of HQT in the prevention and treatment of colorectal cancer. The active components of flavonoids in HQT were searched and screened out by literature review and FAFDrugs4. The related targets of active components were predicted by SwissTargetPrediction, STITCH, and TCMSP. Colorectal cancer-related genes were collected from OMIM, TTD, and GeneCards. The common targets were obtained as the potential targets of HQT in the prevention and treatment of colorectal cancer. Metascape was used for GO function enrichment and KEGG pathway enrichment analyses. Cytoscape was used to construct the protein-protein interaction(PPI) network and "component-target-disease-pathway" network to obtained and analyze core targets and key components. AutoDock Vina was used for molecular docking verification of key components and core targets. The results showed that apigenin, luteolin, wogonin, and baicalein were presumedly the key active components in the prevention and treatment of colorectal cancer, and core targets included TP53, AKT1, VEGFA, PIK3 CA, and SRC. The key KEGG signaling pathways mainly involved PI3 K-AKT, AGE-RAGE, p53, NF-κB, Wnt, Hippo, and calcium signaling pathways. Further molecular docking results showed that four key components showed strong hydrogen bonding ability with the five core targets. This study preliminarily reveals the pharmacodynamic material basis and potential mechanism of HQT in the prevention and treatment of colorectal cancer and provides a theoretical and scientific basis for the application of HQT.

ABSTRACT

Objective:To observe the effect of early weight-bearing on the appropriate population with intertrochanteric fracture after surgery. Methods:From April, 2017 to December, 2018, a total of 45 patients with Evans-Jensen type II intertrochanteric fracture and fracture reduction as positive medial cortex support (PMCS) after proximal femoral nail anti-rotation (PFNA) fixation were randomly divided into control group (n = 22) and experimental group (n = 23). Weight-bearing as tolerated (WBAT) was initiated from six weeks after surgery in the control group, and within 48 h after surgery in the experimental group. The frequency of WBAT in two groups increased gradually from three times a day for ten minutes a time to five times a day for 20 minutes a time until clinical healing of fracture. The length of stay, hospital cost, the fracture healing time and the complication incidence were compared between two groups, as well as the scores of Visual Analogue Scale (VAS) and Harris Hip Score at six weeks, three months and six months after surgery. Results:Compared with the control group, the length of stay was shorter (t = 3.716, P < 0.01), the hospital cost was lower, but no significant difference was found (t = 1.540, P > 0.05), and the fracture healing time was shorter (t = 6.248, P < 0.001) in the experimental group. The complication incidence was lower in the experimental group, but there was no significant difference (χ2= 2.198, P > 0.05). Six weeks, three months and six months after surgery, there was no significant difference in the score of VAS between two groups (t < 1.330, P > 0.05). The score of Harris Hip Score was significantly higher in the experimental group than in the control group six weeks after surgery (t = -5.115, P < 0.001), however, no significant difference was found in other time points (|t| < 1.799, P > 0.05). Conclusion:Early weight-bearing within 48 h after PFNA fixation for Evans-Jensen type II intertrochanteric fractures and reduction with PMCS could shorten the length of stay, shorten the bony healing time and promote early recovery of hip function.

ABSTRACT

Objective:To investigate the characteristics of phosphorylated α-synuclein (p-α-syn) deposition in skin nerve fibers and its potential as a peripheral biomarker for Parkinson′s disease (PD).Methods:Fifteen PD patients and 31 age-matched healthy controls were evaluated by Small-Fiber Neuropathy and Symptoms Inventory Questionnaire (SFN-SIQ), who were recruited in the Department of Neurology, the First Affiliated Hospital of Zhengzhou University from June 1, 2017 to August 31, 2018. PD patients were divided into two subgroups according to the main clinical manifestations: bradykinesia ( n=7) and resting tremor ( n=8), and the severity of the disease was evaluated by Honhn-Yahr classification, where 0 to 2.5 indicates early stage ( n=11) and 3.0 to 5.0 indicates middle to late stage ( n=4). Three-millimeter punch biopsies were taken from cervical seven paravertebral area and distal leg of PD patients, while the skin of the distal leg of healthy control group was only taken, followed by immunohistochemistry and double immunofluorescence staining. The intraepidermal nerve fiber density (IENFD) and the deposition characteristics of p-α-syn were studied. Results:P-α-syn immuosignals were observed in 12/15 PD patients while in none of the control subjects, distributed in shape of dots or thread in the subepithelial plexus, dermis nerve tracts, and/or in the nerve fibers innervating sweat gland, muscle arrector pilorum, small blood vessels or hair follicle. The positive rate of p-α-syn deposition in a single site was 6/15 of the distal leg, 7/15 of the cervical, and the total positive rate was 12/15. The IENFD of PD group was 6.85±1.94/mm, which was significantly lower than that of control group (10.45±3.70/mm, t=-3.303, P=0.002), and was negatively correlated with the SFN-SIQ ( r=-0.561, P=0.046). There was no statistically significant difference in the positive rate of p-α-syn deposition and IENFD between patients with tremor and bradykinesia, nor between patients at stage of Hoehn-Yahr 0-2.5 and 3.0-5.0. Conclusions:Cutaneous p-α-syn deposits in the nerve fibers of PD patients, with a significant decrease in IENFD. P-α-syn deposition in skin nerves may be an intrinsic peripheral pathological change of PD, and skin biopsy immunolabelling p-α-syn merits further study as a potential peripheral biomarker of in vivo PD diagnosis.
